Anna Fleming

I have worked as a Case Manager since 2012, specialising in working with people who have sustained brain injuries, normally through road traffic accidents or medical negligence.  I am a competent and dynamic case manager who is able to support clients and their families with their rehabilitation journey.  I work with clients who have on-going litigation claims, and those whose claims have now settled.  I work with both children and adults.

My caseload is made up of a number of complex cases who require rehabilitation input from a multi-disciplinary range of professionals.  My role is to oversee this rehabilitation, driving along the process whilst ensuring that my client’s needs and wishes are central to the process.

Very often my clients need to employ support workers to support their rehabilitation and changed life; I take an active part in the recruitment, induction and supervision of those support workers.

I work primarily with clients who have acquired brain injury including cerebral palsy, but have a good level of experience in working with people with mental health issues, terminal ill health (including Mesothelioma), and learning disabilities.  Due to my social work background, I have worked with a lot of clients where safeguarding issues are prevalent.

I am fortunate enough to enjoy good working relationships with my clients and their families and have worked with a number of my clients for several years now.  I am passionate about my work and am dedicated to the rehabilitation process.

I am the Registered Manager of Kent Case Management Ltd, which is regulated with the Care Quality Commission.  I work with an experienced Case Manager Assistant which allows us to be mindful of costs, dividing tasks as appropriate to clinical need and relevant experience.  We also employ an administrator to support our work.
