Sophie is one of the founder directors of 3HUB. She has over 25 years’ of clinical experience, caring for individuals affected by trauma and injury, who present with complex physical, psychological, and social needs. She has worked across a full range of care settings.
She is the Director of the Care and Compliance team and manages the delivery of care management services and compliance for 3HUB’s regulated clients.
Her career has primarily focused on the community rehabilitation setting. She has a passion for care of older clients with a special interest in tissue viability having spent 2 years as a tissue viability nurse. She has worked in case management since 2008 with a range of clients, both adults and children, supporting them to rebuild their lives. Sophie is sensitive to the needs of her clients and their families, has a great sense of empathy, and uses these attributes assisting them to navigate the care and rehabilitation pathway to achieve optimal outcomes and improve their quality of life.
Sophie has an excellent understanding of multi-disciplinary teamwork. She is skilled at locating and coordinating multidisciplinary teams and guiding clients through the rehabilitation process, including assessment, identification of needs, goal setting, and review.
She has experience of working alongside statutory providers in securing funding for support packages and has extensive experience of recruitment and ongoing management of large care teams for clients living in their own homes.
