Sophie is one of the founder partners of Three Case Management and founder director of 3HUB. She is the registered manager for Three Case Management. She is conscientious, diligent and thorough in her attention to detail.
Sophie has over 25 years experience of caring for individuals affected by trauma and injury who present with complex physical, psychological and social needs across a full range of care settings. Her career has focused on the rehabilitation setting and she has a passion for care of older clients and a special interest in tissue viability having spent 2 years as a tissue viability nurse.
Sophie has a sound understanding of multi-disciplinary teamwork she has excellent skills in placing and coordinating multidisciplinary teams, guiding clients through the rehabilitation process, including assessment, identification of needs, goal setting and review.
She also has experience of working alongside statutory providers in securing funding for support packages including recruitment and on-going management of large care teams for clients living in their own homes.
Sophie has an in-depth knowledge of the litigation process and the role of case management in this.
Sophie is proficient in her understanding of the requirements of the CQC process and how this is applied to case management. She provides mentoring and support to those applying for CQC registration.
