About 3HUB: We are a unique, collaborative, client centred and clinically led case management community. We deliver expert, safe and well governed care and rehabilitation to our clients who predominantly have had life-changing injuries.
3HUB was founded in 2018 by Viv Cooper, Sophie Greengrass and Kaaren Wallace, who between them have decades of professional expertise in health management and rehabilitation.
As their own independent practice grew they recognised an opportunity to support other highly experienced, case managers to progress to independent practice and, as a result, 3HUB was born.
3HUB ensure independent case management practice is safe and supported whilst enabling 3HUB members to excel at what they do best – helping clients rebuild their lives.
Case Management Services You Can Trust
Our Case managers are all regulated health care professionals who comply with due diligence protocols which include agreed standards and timeframes.
We provide Initial Needs Assessments (INA’s) and ongoing case management with Adults and Children who may be experiencing some of the following:
- Brain Injury
- Birth Injury
- Spinal Cord Injury
- Multiple traumatic injury
- Complex orthopaedic injuries
- Challenging Behaviours
- Post-Traumatic Stress Disorder (PTSD)
Governance & CQC
We are CQC Registered and Regulated. The 3HUB Community have bespoke clinical templates, policies and processes that reflect best practice and comply with CQC regulations. Please see our CQC registration information here and you can give feedback via our contact page here or via CQC here.
We have implemented rigorous governance structures, to reflect the unique role of the case manager and to ensure community members remain professionally supported through regular peer governance group meetings. All our members have access to our specialist Safeguarding Consultant to support the needs of our complex clients.
We support our members to become accredited with all the major trauma pathway organisations throughout the UK.
